Classes
start promptly at 6:30 pm
If you have checked YES
to any of the
questions on the medical questionnaire, a
physician must sign a medical form stating it is
okay for you to participate in the Scuba Diving
Program. If you have a YES on your Training
Record,
without written permission by a physician you will
not be able to participate in the certification
course.
Please call the store if you have any questions or
to obtain a medical form for your doctor to sign.

Bathing Suit, Towel, Completed Chapter
Quiz, Mask, Snorkel, Fins and Boots
