Mask, Snorkel, Fins and Boots; and any other gear you have purchased 
  Bathing Suit and a Towel 

IMPORTANT REMINDER
If you have checked YES to any of the questions on the medical questionnaire, a physician must sign a medical form stating it is okay for you to participate in the 
Scuba Diving Program. 
If you have checked YES and do not have written permission by a physician 
you will not be able to participate in the certification course.
Please call the store if you have any questions or need to obtain a medical form for your doctor to sign.
